Anxiety is one of the most common reasons people try brain training. Here is an honest, plain-language guide to the research on neurofeedback and heart-rate-variability training for anxiety, with links to every study.
with 484 participants were pooled in a 2017 meta-analysis of heart-rate-variability biofeedback for stress and anxiety.
HRV biofeedback outperformed control conditions with an effect size of 0.83 in that meta-analysis, a large reduction in self-reported anxiety.
In a 2020 trial, anxiety fell after real SMR neurofeedback but not after sham, and cortisol levels moved the same way.
of alpha training lowered anxiety in adults with generalized anxiety disorder, and scores kept improving four weeks later (2021).
in a randomized trial where frontal alpha training reduced anxiety and negative mood compared with an active control (2017).
Neurofeedback and HRV training use no medication. They can sit alongside therapy or medication; talk with your prescriber before changing either.
Anxiety often shows up as a nervous system stuck on high alert: extra fast beta activity, too little calming alpha, and a heart rhythm with little flexibility. Neurofeedback reads these signals in real time and rewards the brain, through a game, video or sound, whenever it drifts toward a calmer, steadier state. With practice, that state becomes easier to reach on your own.
The most-researched approaches are:
- Alpha training: strengthens the relaxed-but-alert rhythm linked to calm focus.
- SMR training: builds the 12-15 Hz rhythm tied to steadiness and better sleep.
- Alpha asymmetry training: rebalances frontal activity linked to worry and negative mood.
- HRV biofeedback: slow, paced breathing that trains the heart and nervous system to recover from stress.
The evidence is strongest for HRV biofeedback. EEG neurofeedback trials for anxiety are encouraging but mostly small, so we show both honestly below.

Training at home: a soft EEG cap reads brain activity while a show or game gives instant feedback.
META-ANALYSIS | Psychological Medicine. Pooled 24 studies and 484 people using heart-rate-variability biofeedback and found a large drop in self-reported stress and anxiety compared with control groups.
SYSTEMATIC REVIEW | Advances in Experimental Medicine and Biology. Biofeedback beat wait-list for anxiety disorders but not active treatments, and most studies were rated weak.
SHAM-CONTROLLED TRIAL | Neurophysiologie Clinique. Real SMR neurofeedback lowered anxiety and cortisol more than sham, though placebo effects likely also played a role.
RANDOMIZED TRIAL | Brain and Behavior. Ten sessions of parietal alpha training reduced anxiety, depression and insomnia scores in 26 women with generalized anxiety disorder, with gains still growing at four weeks.
RANDOMIZED TRIAL | Behaviour Research and Therapy. Frontal alpha asymmetry training reduced anxiety and negative mood in 32 women compared with an active control training.
CONTROLLED TRIAL | Cognitive Neurodynamics. In healthy adults with anxiety, SMR neurofeedback raised the targeted rhythm, and larger gains tracked with lower anxiety.
- A skills-based, non-medication approach you can use on its own or alongside therapy and medication, with your provider's guidance.
- Non-invasive: sensors only read brain and heart signals. Nothing is sent into the brain.
- You learn what calm feels like in your own body, a skill you can use in stressful moments.
- Engaging: feedback comes through games, videos and sounds.
- At home with NeuFidelity: unlimited sessions, monitored remotely by our team.
A FEW HONEST CAVEATS
- Results vary. Some people feel calmer within weeks and others notice little change.
- Most EEG neurofeedback trials for anxiety are small, and placebo effects likely play a part.
- Reviews find biofeedback beats no treatment but has not yet beaten other active treatments.
- It does not replace diagnosis, therapy or crisis care. Never change medication without your prescriber.
HOW NEUFIDELITY APPROACHES IT
Every program starts with a Brain Map and Trait Map. Protocols are matched to your own patterns, adjusted remotely as your data comes in, and checked with repeat assessments so you can see whether training is working for you.
FREQUENTLY ASKED QUESTIONS
Can neurofeedback replace anxiety medication or therapy? No. Many people use it alongside both. Talk with your provider before any change.
How soon might I notice a difference? Some studies saw changes within 5 to 10 sessions, but most programs run longer. Your plan depends on your brain map and progress.
What is HRV biofeedback? Paced breathing guided by your heart rhythm. It has the strongest research base for stress and anxiety.
Is the evidence settled? No. It is promising but still growing, which is why we personalize training and measure your results.
Every NeuFidelity program starts with a Brain Map and Trait Map, so your training is matched to your own brain. Talk with our team to see whether it fits you.
NeuFidelity provides non-invasive brain training. It is not a medical treatment and is not intended to diagnose, treat, cure or prevent any disease. Individual results vary. Please consult your healthcare provider about any medical condition.
